Reporting tool for aviation safety occurrences
A Czech university has developed a web-based tool to support the reporting and analysis of aviation safety occurrences. The tool allows classification according to International Civil Aviation Organization as well as causal event chain modelling and analysis. The tool can be used directly by aviation organization to create occurrence reports compatible with latest EU regulations. The researchers look for industry partners to help in further development and commercialization of the solution.

Aviation organizations are mandated to report specified safety occurrences from everyday operations. The reporting scheme in EU defines format of the data to be provided, in essence based on the International Civil Aviation Organization taxonomies. However, the taxonomies are very robust and not user-friendly. In result, reporting forms are often extensive and aviation organizations tend to underestimate or simplify the reporting process and report incomplete, inconsistent or incorrect data. Consequently, the statistics and analyses made using such data is limited and do not allow establishing the full picture of safety performance in the industry. Newly developed solution is a toolkit of web applications that improves quality of the gathered safety data. The reporting tool provides structured reporting form usable for both mandatory and voluntary reporting of an occurrence. In addition, it allows recording new type of information, namely the chain of contributory factors and final event outcome. The application also includes a basic analytical module, that provides several statistics, including causal chain graph, allowing to identify critical paths of safety events that can potentially lead to an incident/accident. Another part of the toolkit is a web service providing similar functionality as the reporting tool and is meant for integration of the main features of the solution into an established safety management software. Several profiles of the web application, customized for different types of aviation organizations, are available.
Advantages and innovations
The toolkit is based on ontology and conceptual modelling of the aviation domain, which is a novel approach to ensure maintainable and manageable terminology. This is the core innovative process which is aimed to assure that data are reported in consistent way. The solution is based on the so-called aviation safety ontology, which remodels International Civil Aviation Organization taxonomies and and fits them into a knowledge framework capable of formal reasoning as well as adhering to common sense top-level ontologies. A specific innovation of the tool is the chain designer. This is a module that allows modelling of chains of contributory factors and event types, i.e. it makes it possible to express chronological order of factors and events occurrence so as expressing the causal (or possibly mitigation) relationships between them. This allows the organizations to monitor not only negative trends, but also patterns in the data and positive relationship between contributory factors and final outcome. In the short term run, the main advantage of the toolkit for an organization is easiness for reporting International Civil Aviation Organization - compliant causal chains and their analysis. In the long term run, the toolkit pays of for safety knowledge sharing within an organization and maintenance/management of a custom organization International Civil Aviation Organization - compliant vocabulary.
